I have Cablevision iO with a SA Explorer 4200 and had the same
problem. My manual said to clear the pin enter four zeros. It worked
for me, so give it a try. Hopefully it's universal across it whole
line.
0000 or 9999 are only default PINs that the box is originally set to.
In order to clear the PIN you need to do a hard boot; just disconnect
the "cable in" for about a minute, plug it back in and let it reboot,
the PIN should be cleared and returned to the default (0000 or 9999).
If that doesn't work, then you'll have to give them a call.
That's not going to work because it's so simply defeatable. Think about it.
Any 10 year old kid wanting to watch skinamax at night could unplug a box.
Once a PIN has been set, it can only be cleared by the billing system, i.e.
Cabledata, or by the box controller. It is security after all.

Guess who I work for?? Maybe the cable company as a technician and
that is how we do it on our SA and pioneer boxes. And there have been
cases of kids doing that, but not many since most don't know about it.
That is they way they come from the manufacturer.

I suggest you go down to Atlanta and take the SA DNCS A&O course because you
don't know what you're talking about. When a set top is initialized, the SA
controller sends a UNconfig broadcast message to the set top which among
other things, contains the default PIN (usually 0000, you got that part
right). But it also can be whatever the system operator sets the default PIN
to in DHCT configuration. PIN's are stored in the set top's NVRAM. Once the
user goes in and changes the PIN in the general settings menu in the set
top, you can power cycle that box until the cows come home and it is not
going to clear the PIN out. Either a billing system hit (reset PIN command)
needs to be sent out to the box by a CSR or the an addressable UNconfig
message can be sent out to the set top directly from the controller in the
headend. All the set top power cycle does is forces the set top to resign on
to the SA network so it gets the UNconfig broadcast message faster. But
there still has to be a config message sent to the box in order to clear a
users PIN. Incidentally, this is one of the reasons that all set tops need
to be restaged when they are returned to the warehouse from the field. That
and clearing recording timers and outstanding PPV purchases.
